Documentation Standards Policy

Policy Statement:

All Licensed Practical Nurses (LPNs) must document client care in a timely, accurate, complete, and professional manner, in accordance with the Association of New Brunswick Licensed Practical Nurses (ANBLPN) Standards of Practice and applicable legislation. The DARP charting format is the required method of clinical documentation for client encounters.

Purpose:

To ensure nursing documentation consistently reflects a legal, professional, and client-centered record of care, using the DARP framework to support clinical decision-making, continuity of care, and accountability.

Scope:

This policy applies to all LPNs employed in any setting, clinic, community, or mobile practice, where nursing documentation is required, especially in foot care and community nursing contexts.
